Please Read :	I have verified that I have sufficient balances in my paid leave account(s) to cover the leave requested above. In the event that I do not have enough hours accumulated in the appropriate category to cover this leave, I understand that my vacation balance will be docked and, if no vacation hours are available, my pay warrant will be docked and my supervisor notified. Sick, personal necessity and personal business leave do not accrue throughout the year. Vacation is accrued monthly. Balances can be verified by contacting the department's Executive Assistant or by consulting your most recent pay warrant stub.
