Plan Selections and Changes
Important information for Employees: All plan choices are the same as last year; rates have changed.
If you are covered through another health plan and therefore do not wish to enroll in a SISC medical plan (medical, dental and vision), the Waiver of Anchor Bronze Enrollment (WABE) premium option is now available. You can find the form and more details below.
- If you are happy with your plan and the dependents on it, you do not need to do anything!
- If you wish to add/remove a dependent please complete the SISC Membership Change Form before continuing.
- If you are full time, have another medical plan and opting to decline coverage you MUST complete the WABE Employee Election Form.
- If you wish to change your plan options click HERE to Start Form – Login Required (Use your SLOCOE Email address and password)
How to log into Forms
- Enter your SLOCOE Email in the User name field
- Enter your password in the Password field
- Click Sign In
- Note: If you are not on your computer or device then make sure to select the check box below the Sign In button
